Pursuant to Order of this Court dated February 10, 2006, you are hereby notified that on February 10, 2006, a petition for adoption and a motion to terminate parental rights was filed in this Court by the adoptive parent of the above-named child asking the Court to terminate your parental rights to the above-named child.
